The Human Factor in Population Health Management

The healthcare industry is moving toward population health management. We see that quite a bit, but it’s such a broad concept and construct, that if you put your toe in the water in this field, you may likely earn the job as a population health manager.

Earlier this year, Xerox polled a group of C-level healthcare executives and asked them to provide their perspective on population health management, starting with how it should be defined – and asked them to rank five components of a definition in order of significance.

What is population health management? Definition components were ranked in the following order:

  1. Facilitates care across the health continuum
  2. Supports providing the highest quality of care at the lowest cost
  3. Uses actionable insight for patient care based on a variety of data
  4. Targets a specific population of individuals
  5. Enables patient engagement

In my opinion, Population Health Management is the ability to manage clinical information, roles and workflow functions at every level – from administrative to quasi-clinical to clinical [see figure 1 below] – and deliver actionable insights that enable payers and providers to improve both clinical and financial outcomes.

There are a number of players today that claim to provide population health management services; however, very few have the people power and the level of interoperability needed to support workflow at these various care levels. In fact, these solutions are often provided by a number of different vendors, creating disintegrated, disjointed technology that works in silos – limiting the healthcare service provider’s ability to see the full picture.

Take payers, for example, who have long applied data analytics from billing claims in an attempt to improve quality and reduce costs. Payers leverage this data as a way to stratify populations or groups within a population and identify at-risk groups so they can provide alternate methods of care. While data from billing claims can be used to support and measure the efficacy of the population health management goals in place, the data is not always a clear reflection of the patient’s health situation. Further, the payer’s ability to have a real impact on population health is limited because they don’t have the appropriate relationships with the patients, nor the cross-clinical information sharing capabilities which minimizes their effectiveness in understanding the complete picture.

In the future of population health management, it will be critical to connect these layers where there are specific roles defined. It will require business engineering that integrates the human factor into the technology solutions and end-to-end solutions that support the full population health management workflow with interoperability, information-sharing and real-time data analytics.

Technology partners have an important role to play.

Almost every one of these actions and process components are likely to be able to be automated or enabled by technology to some degree, but technology alone isn’t a solution.

Technology can facilitate cross-physician and patient communication and increase collaboration through information-sharing. It can provide the foundation for capturing clinical data, providing 360-degree patient views and analytics that can quickly and accurately identify at-risk patients. Consumer technologies are equally as important – from detecting and monitoring chronic diseases to driving a new and higher-level of medication adherence.

Automated tools can support workflow, save time and help manage risk as the industry transitions from fee-for-service to value-based care, but there must be some level of insight into the human-centric workflow.

The path to improving care outcomes and reducing costs for an entire population resides within the ability to reduce inefficiencies and improve the work itself.

As the industry continues to take steps toward achieving population health management, it will be equally important to ensure a seamless integration with support functions such as patient acquisition, patient & provider service, claims management, wellness services and revenue cycle management. It will require more machine-to-machine conversation and technology that supports people and processes – not the other way around. Integrated data from consumer technologies will play an equally important role – and will drive a new level of medication adherence and improved care outcomes that we haven’t seen in the past.

But achieving this level of integration and support will require payers and providers to consider a smaller group of highly capable sourcing partners, who are able to provide complete, end-to-end solutions.

Subsequently, this shift in payer/provider focus will have a significant impact on how business process outsourcing vendors develop new solutions and go to market. By developing and integrating more cross-industry solutions and considering new outcome-based compensation for services and solutions that align with value-based care, BPO vendors can enable a differentiated experience that is critical in the new healthcare economy.

800 Million Lives Per Year… AWS and Siemens Collaboration

AWS (Amazon Web Services) has a wide scope of satisfied customers in the field of healthcare. Siemens healthcare diagnostics is one of them. The company specializes in advanced human health and they are serving in diagnostic labs, hospital labs, physician labs using their lap analysis machines, intelligent software, and corresponding diagnostic machines.

With their services they are able to save 800 million lives every year with 9 billion tests. Health is one of the main issues of today’s world and the sad fact is that everyone in 3 people are diagnosed with cancer. Diagnosis is sometimes not enough because there are sub types of cancer, or the treatment for that test is not available for that type of sickness.

Siemens thus initialized a personalized program test to diagnose a subtype of illnesses and provide that information to the concerned party. This could enable care takers to personalize and customize treatment plans to increase the ease and lessen the discomfort of the patient.

For this purpose, they required large data analysis about the illness of different types, and they had to deploy analytics software within their environment. AWS not only provided them with the required solution but also with the automation of workflow. This enabled Siemens to concentrate on their business drive rather than technical constraints. The solution was purely seamless and secure. The data from the lab was automatically stored in the cloud and passed afterwards to analytical applications that provide results along with the patient history, which is securely saved to the custom application the customer has access to. The access is purely secure. All the applications got to be secured, automated and within the budget.

Siemens required active logging, security compliance, network encryption. The company wanted to know what is going on in the infrastructure of the AWS and what the activities on the daily basis are. Siemens used CloudTrail to monitor activity in the cloud and to be aware of all the system activities. Companies like Siemens want their technical infrastructure to be highly scalable. This is the reason that AWS takes care of this feature very much. For this purpose, Siemens used many services of AWS like Elastic Load Balancing for managing the traffic of data, Amazon Simple Queue Service, Amazon Elastic Cloud Computing, Amazon ElastiCache, IAM, CloudWatch, and CloudFormation.

RDS was used to handle heavy workloads going in and out of the system during the research. AWS made the Siemens technology more agile because it automatically adjusted according to the requirements of the users. Most importantly, the solution enabled the accomplishment in much less cost as compared to the previous infrastructure. This is the reason that the company was able to drive more technical innovations in their environments for the sake of more efficiency and automation. Thus, Siemens attained their desired milestone which was to attain security, agility, scalability and at less cost, said Andreas Heid.

AWS and Siemens collaboration proved to be a great success which not only solved business related problems of Siemens, but also paved the way for further advancements so that both parties can go a long way in the field of technology integration with healthcare for the benefit of mankind.

2016’s Healthcare Web Design Trends

The healthcare industry and Internet marketing have at least one thing in common: both are changing rapidly. Healthcare practice websites built even two or three years ago may be seriously out of date in 2016 and require significant upgrading to continue being effective for patient communication and appointment scheduling. Here are key trends to incorporate in Web design going forward.

Responsive Web Design

Responsive Web design is a set of techniques used to make a website adjust automatically for optimal display on desktop monitors, tablets and mobile phones. Now that mobile Internet access exceeds desktop access — and the gap is widening — every healthcare website must deliver mobile users a high-quality experience. Mobile Internet users are likely to access healthcare sites “on the run,” to schedule appointments, confirm appointments, get driving directions and do research.

Websites built on solid CMS platforms such as WordPress can usually be made responsive by converting to one of their responsive templates — a fairly inexpensive, easily implemented task. For a fully custom website or one with complex content, the conversion will likely entail a complete redesign; in such a case, selecting a Web designer with demonstrable responsive design experience and expertise should be a key component of the vetting process.

Mobile-Driven Page Layouts

  • Tell stories. Vertical scrolling is OK. In the past, websites tried to cram all of the important information “above the fold,” to keep desktop users from needing to scroll down. Here is a good example of an effective storytelling technique on a home page.
  • Use “sticky” navigation to keep the main navigation in view as users scroll down. This is often done with a “hamburger” menu at the top of a page; here is an example of user-friendly, sticky navigation. For medical practices, using a sticky phone icon alongside the menu is also very important, making it super simple for users to call no matter where they are on the website.
  • Mobile Internet users are attracted to and motivated by visual communication. Replace bland stock photography with high-impact, custom images; add custom graphical elements such as charts, graphs and simple infographics; add short (1- to 3-minute) video.
  • Make contact forms simple and large enough for mobile users to fill out, as they may not have the patience or dexterity to deal with numerous, hard-to-view form fields.

Local SEO

If SEO is part of a healthcare organization’s marketing strategy, implementing up-to-date local SEO techniques will be quite important in 2016. Over the last year or two, Google has made significant refinements and changes to its local search results and search algorithm; today’s local SEO focus has changed as a result. With regard to website design and local SEO:

  • NAP (Name, Address, Phone) citations are critically important. Make sure the organization’s name, address and phone number are displayed consistently on every page of the website.
  • Add or add to a user review page. User reviews have grown in importance for local SEO; having reviews on the website sends Google a signal that the healthcare organization is competent and credible.
  • Add relevant, local-oriented content to the website — for instance, mentioning the geographic area of coverage, neighborhoods served, and commentary about the nature of the local market.
  • Attend to the basics. Content site-wide must be unique, relevant, engaging and informative. Each website page must have a unique title tag incorporating a strategic keyword specific to that page. Here is a useful on-site SEO checklist that covers all the necessary points.
  • Last but not least, a mobile-friendly website is also very important for local SEO — something covered in detail earlier.

Review the Website Annually

Going through a complete website overhaul is an expensive, time-consuming and often painful experience. Unfortunately, with the state of Internet marketing being what it is, an overhaul may be necessary every three to five years — unless a medical practice reviews its website design annually to identify and implement tweaks and upgrades. The old adage “out of sight, out of mind” is equally ill advised in healthcare and website design.

Journey Mapping – Three Key Insights You Have to Have (Part-II)

In the last post, we talked about how Journey Mapping can save you from making false assumptions about your customers. And trust us. Every single client we’ve ever worked with has some assumptions that are surprisingly wrong.

It’s not a crime. But it can be disastrous, depending on how wrong you are, how much money you’ve expended in the wrong direction, and how late in the game you find out.

These pivotal insights will help you avoid risk and develop truly human-centered products that succeed.

Insight #1: How do they think?

One of the many advantages of journey mapping is the ability to create a high level mental model that chronicles how people really think while going through the experience.

You’d be surprised at how often a company discovers that they don’t know their customers as well as they thought they did.

For instance, we just built a website for a large company that helps people find health insurance through their employer. The company believed that the best way to approach this was to treat it as a retail shopping experience, where customers could choose the insurance options they wanted and place them in a shopping cart. So we said, “Let’s see if this is true.”

Within days, we created an experience and tested it out with users. What we heard was an overwhelming, “NO! I’m not shopping. I’m getting insurance. Why do I have a cart? I don’t want a cart.”

By getting a reality check and revising the mental model, our client was able to shift their approach and avoid building a website that was nothing like what customers wanted.

Where do they stumble? What ticks them off? That’s where your company can be the hero.

Insight #2: What do they do?

The journey map also highlights various touch points that the user interacts with along the way and what happens at those touch points – whether it’s searching online, talking on the phone,  meeting face-to-face,  communicating via text or email, etc.

These touchpoints include interactions with your competitors, other companies, products and people to give you the birds eye view of what is involved. Who do they come in contact with? What channels are they using? How are they gathering information along the way?  Just one key touchpoint might be the catalyst for a solution that  better addresses the needs of your customers.”We know people are having trouble right here, what are we doing about this next step?”

Insight #3: How do they feel about it?

It’s not enough to understand how they go about getting from A to Z on their journey. The gold lies in understanding and tracking their emotions. Where are they frustrated? Where are they confused? Where do you lose them? Also, where do they feel a sense of accomplishment or enjoyment? How can you build on that?

Think You Know Your Customers? (Part-I)

In the movie Field of Dreams, Kevin Costner hears a voice that tells him, “If you build it, they will come.”

Over and over, he hears this voice from out of nowhere until he eventually decides to plow down a good portion of his cornfield, throwing out much of the year’s harvest, and risking everything to build a little baseball field on his land.

Who are “They” and why will they come? No one knows.

But come they do, in long lines of cars stretching as far as the eye can see, night and day. So, it all worked out.  That’s the good news.

The bad news is that too many companies are using this as a business strategy.

They build something they feel is the right thing and are shocked to find out nobody is coming.

A report from CB insights last year on The Top 20 Reasons Startups Fail cited “No Market Need” as the #1 reason for failure. “Poor product” was #6 and “Ignore Customers” was #9.  How ignoring customers ended up lower on the list is a little bit of a mystery given that this is  almost always what leads to developing products that the market doesn’t need. But you get the point.

If you’re reading this and feeling safe because you don’t work in a startup, not so fast. The corporate world is also rife with failed products, services and programs. It’s just harder to document.  The real question is: Do you want yours to be the next?

Of course not.

What can you do to avoid building it and having no one come?

And what single step can you take right now to CYA in 2016?

Start by checking your assumptions.

What exactly is Journey Mapping?

Remember the old saying, “If you want to understand a person, you need to walk a mile in their shoes”?  That’s journey mapping in a nutshell.

Journey mapping is a group exercise that seeks to understand what it’s like to walk in your customers’ shoes throughout their journey to complete a goal they might have.

The goal might be to find the best deal on a car online, or to choose a banking product or sign up for health insurance. One of the most well-known examples of journey mapping was actually a map of what it was like to be a customer traveling through Europe by train, documenting the customer experience — highs, lows, frustrations and likes —  for Rail Europe. Although journey mapping has been around for the better part of a decade, its use for healthcare organizations is relatively new. But the advantages in getting something to market that is not only timely, but also on target with what the market wants are undeniable.

When we do a journey mapping  workshop, we create a large visual representation of the journey a person makes from the beginning until they accomplish their goal.  This map hangs on the wall and guides your team/organization as you move forward. Often, teams will refer back to the map to make sure they are not overlooking an important point or operating on an old assumption.

From this map, you and your team gain a deeper understanding of how your customers see things. You can align people from various internal groups who have been thinking about the product or service in very different ways, get everyone to share the same mental model and speak the same language about the customer experience on this journey.

How is Health Important in the Business World?

The healthcare industry is massive and a big part of the modern world. People need to take care of their health at all times. In today’s piece, I explore how health plays an important role in the business world:

Health and Safety At Work

There are many cases all over the world in which someone gets injured at work. This is usually down to poor health and safety in the workplace. If this is the case, it falls on the business owner to be responsible for the accident. As a result, you may have to pay lots of money to cover the health care costs for your injured employee. This could be a major problem for some business owners. Your employee could go through a private health care service and end up costing you tonnes of money.

But, there is a way that you can prevent this from happening if you’re ever in this situation. Firstly, you can start attending health and safety seminars. This way, you can learn how to keep up with current laws and standards. Doing this will minimize the risk of accidents happening at work because you’re providing a safe working environment. Plus, if it meets current health and safety standards, it’s hard for it to be your fault if someone gets injured. You’ve done everything to ensure that your workplace is fit a safe place.

Secondly, you can provide healthcare for your employees as a benefit. So, if anyone gets hurt or sick, you’re already providing them with support to get better. It means that, should someone get into an accident at work, your company is already covering their health costs. Sure, you’re still going to have to pay money. But, it’s on your terms. You’ve paid for company healthcare and won’t have to rack up additional costs that their private care might issue.

A Lack of Healthcare is Bad For Business

This brings me nicely to my next point. Not providing healthcare services to your employees is bad for business. As I’ve already mentioned, you can end up spending a lot of money if you don’t provide healthcare. But, this isn’t the only way that it can be bad for business.

For starters, it can damage the image of a company. If a company doesn’t provide any health-related benefits to employees, it looks bad, especially if all its rivals and competitors are providing things like a company healthcare policy and health insurance. It makes the business look like it doesn’t care for its employees. The business looks like it would rather save money than do what’s right and provide healthcare benefits to members of staff. Anyone with a business brain can tell you that reputation is very important to the success of a company. A bad image will lead people to avoid a business. People want to do business with companies that have a good, clean, image.

There are plenty of healthcare providers out there that are designed for business use. They can give you cover over so many things. And, they’re affordable too. There’s no excuse for a business not to supply this type of benefit to their workers, especially if you have many employees and work in a place with high risks, like the construction industry. If you’re a business owner, I highly recommend you look at the various healthcare schemes out there.

Employee Health is Key For the Health of a Business

A healthy business is one that makes a lot of money and beats all its rivals. An unhealthy one will struggle to make money and fall behind by the wayside. There are many things that contribute to a healthy or unhealthy business. One of the biggest of which revolves around health itself. Yes, the health and wellbeing of employees is key to a business. You may be confused as to how the two relate to one another, but allow me to explain.

You see, a business relies on employees to do a lot of work everyday. Think of it as like a machine with lots of parts moving together. If just one of these parts breaks, the machine won’t function properly. Now let’s look at it from a real-world point of view. A business has lots of employees working together as one. If one of them falls ill or gets in an accident, they may be unable to work. So, the business loses one member of the team, and it struggles to function as well as it should. People are having to do extra work to compensate for the employee that’s ill or injured. As a result, it could lead to increased stress levels, and more employees are struggling to work to their maximum ability. All of this combines to have a negative effect on the business. People aren’t able to work as hard and less stuff will get done. The company becomes less productive. Less work being done means less money being made. In turn, the health of the business suffers.

Unfortunately, sometimes there is nothing you can do to stop someone getting ill. We all go through periods where we might be unwell and get the flu. Or, someone might get an even worse illness that requires months to recover from. In this situation, I relate back to the earlier point of providing healthcare benefits to staff. If a business does this, it can, at least, help get the sick person back on their feet a lot quicker. You can provide them with quality healthcare that will help them recover. It’s also a good idea for business owners to do what they can to reduce stress at work. Stress is the cause of many health issues that lead to dips in productivity. Making a workplace less stressful is important to employee health and the health of a business. As a business person, you will undoubtedly want to grow your businesses profit margins, to do better against KPIs and to increase consumer confidence. Johnson and Johnson Turns to Amazon Web Services for Hybrid Cloud

Johnson and Johnson have diversified operational area in the medical field. It is the largest pharmaceutical, healthcare, medical device manufacturer, working in bioinformatics and innovating drugs. It has over 270 operating companies that operate 60 countries across the globe.  Johnson and Johnson have a decentralized model, having small and midsized companies in the circle. As it has a heterogeneous distributed system, it is purely physical in nature and performs provisioning in weeks. These systems were having low utilization of resources and low financial transparency, while business partners and customers were demanding for something better as far as the technology drive is concerned.

They first took a step to virtualize at the compute level and do some process improvements, but that was not enough to meet the technology needs and was not up to the mark for the upcoming challenges. Johnson and Johnson took a step to deploy hybrid cloud strategy. They decided to run most critical workloads on a Amazon Web Services’ (AWS) public cloud and some legacy applications on their data centers. The decision was fruitful and it helped them gain business efficiency and strength of their IT environment by making it more automated and agile. The hybrid cloud infrastructure has allowed the IT team to reduce the footprint of physical servers, automate IT, and gain business efficiency, said Zalem.

Hybrid clouds are fundamentally defined as bridging a public cloud with a private one. While many people think about private clouds running on customers’ own premises, Amazon AWS takes a slightly different view. Amazon AWS offers virtual private clouds (VPC), which provide customers with a logically isolated section of the company’s public cloud. “Our VPC has really become an extension of our customers’ own data center topology,” said Jassy.

The company currently leverages theAmazon AWS Cloud to run 120 applications and for its big data architecture, allowing it to complete highly intensive and complex data modeling. Next, the company plans to triple the number of applications it hosts on AWS and launch 25,000 Amazon Workspaces cloud-based desktops for its consultants and employees to use around the globe. It will also help you promote self-service and bring your own device (BYOD).
